I’m Jasmine O’Sullivan, Director of Intricate Care Pathways, and I bring over 12 years of experience working across the community, disability, and government sectors. Throughout my career, I’ve worked closely alongside agencies such as the Department of Communities and Justice, NDIS, Housing, the Department of Education, mental health services, and a range of mainstream and community providers.
My professional background spans leadership, senior case management, counselling, crisis intervention, and support coordination. I am a Certified Support Coordinator and an accredited neurodivergence coach, supporting children, teenagers, and adults to build confidence, capacity, and meaningful independence.
My approach is deeply person-centred, grounded in both professional expertise and lived experience. As a parent of children with diagnosed Autism Spectrum Disorder (ASD), and with my own late diagnosis of ADHD, I understand firsthand the challenges, strengths, and complexities that come with navigating systems and supports. This perspective allows me to connect with individuals and families in a genuine, empathetic, and practical way.
Outside of my professional life, I’m a proud mum to two daughters aged 18 and 20, and I’ve been with my partner for over 20 years. Our home is full of life with three dogs, two cats, and a lizard so I understand the importance of patience, adaptability, and a good sense of humour!
I am also an active member of multiple professional networks and memberships, ensuring I stay current, connected, and continually improving the way I support participants and families.
